Intensive Intervention/Case Management (Pike County Board of Education) East Central Mental Health-Mental Retardation, Inc. through an agreement with the Pike County Board of Education provides a Case Manager/Community Outreach Specialist.The Case Manager/Community Outreach Specialist provides intensive intervention/support/outreach to students identified as At-Risk by the Pike County Board of Education as determined by the Student Discipline Council (SDC) designated staff and/or by long-term placement in the Alternative Learning Center (ALC), to assist with training/intervention and to assist with identification of services for health intervention.
Services provided by the Case Manager include the following:

Needs Assessment

Coordination with and/or referral to other Community Service Providers (i.e. Juvenile Court, DHR)

In-home follow-up services to monitor condition set by the individual school disciplinary council.

Monthly reporting to the disciplinary council/At-Risk Coordinator

On-going consult/collaboration with appropriate school personnel (i.e. teachers, principal, nurse, counselor, etc.)

Participation in meetings related to At-Risk (i.e. IEP, multi-agency planning, etc.)

Assistance with Truancy Intervention

Assistance with assessment identification of services for health intervention
